Omega-3 Dosage: What Clinical Trials Actually Use
The most important omega-3 dosage point: focus on EPA+DHA content, not total fish oil grams. A typical 1,000mg fish oil capsule contains only 300mg EPA+DHA (30% concentration). To reach a therapeutic dose, most people need 3–6 capsules of standard fish oil — or fewer capsules of concentrated rTG or ethyl ester formulas.
Dosage by Goal
General health maintenance: 1–2g EPA+DHA/day. The American Heart Association recommends 1g EPA+DHA for general cardiovascular health. This is the minimum dose with consistent clinical support across outcomes.
Inflammation reduction: 2–4g EPA+DHA/day. Clinical trials targeting inflammatory conditions (joint pain, post-workout inflammation) consistently use 2–4g EPA+DHA. The anti-inflammatory effects of omega-3 are dose-dependent — 1g/day produces modest effects; 2–4g/day produces more robust results.
Triglyceride reduction: 2–4g EPA+DHA/day. FDA-approved for reducing very high triglycerides at 4g EPA+DHA/day. Studies show 25–30% reduction in triglycerides at this dose. If managing high triglycerides is the primary goal, aim for 4g EPA+DHA.
Mental health / mood: 1–2g EPA+DHA/day, with a higher EPA:DHA ratio (at least 60% EPA). Research on depression and omega-3 consistently shows EPA (not DHA) is the active component for mood. Choose formulas with 2:1 or higher EPA:DHA ratio for mental health applications.
Cognitive function: 1–2g DHA-dominant omega-3/day. DHA (not EPA) is the primary structural fat in neurons and synapses. For cognitive support, choose formulas with higher DHA content. Pregnant women and infants need DHA particularly — 200mg+ DHA/day recommended during pregnancy.
The Form Matters: rTG vs EE vs Triglyceride
rTG (re-esterified triglyceride): Best bioavailability (+50% vs EE). The form in quality supplements. Look for rTG on the label.
Natural triglyceride (krill oil, some fish oils): Good bioavailability, phospholipid form in krill has excellent absorption.
Ethyl ester (EE): Standard pharmaceutical form (Lovaza). Absorbs well with fat-containing meals but inferior to rTG fasted.
Free fatty acid: Available, expensive, highest bioavailability.
Standard fish oil (triglyceride): varies widely in quality and oxidation level.
Practical: How Many Capsules Do I Need?
Check your product’s label for EPA+DHA content per serving. For 2g EPA+DHA/day target: if your capsule has 600mg EPA+DHA → 3–4 capsules/day. If 1,000mg EPA+DHA per capsule (concentrate) → 2 capsules/day. rTG-form concentrates (e.g., 2,000mg omega-3 per 2 capsules) are the most practical option.
